How We Provide Care

We thoroughly evaluate each person’s needs and from there, we strive to design and deliver a comprehensive care plan that can address all of their needs. Delivered by trained and competent home health care professionals and paraprofessionals, our services include:

Registered Nurses and Licensed Vocational Nurses

Our Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) will evaluate and assess patient medical needs and render treatment, instruction, and teaching in accordance with the plan of care which is duly approved by the attending physician.

Registered Dietitian

Our Registered Dietitians will assist the physician and other agency personnel in evaluating the dietary needs of the patient.

Physical Therapists

Our Physical Therapists will assess the patient’s physical condition and physical therapy plan in order to help the patient with their activities, alleviate and prevent pain, safe ambulation, transfer, balance, gait training, and fall prevention.

Occupational Therapists

Our Occupational Therapists will work in conjunction with other services for the patient to adapt functional skills that are necessary for activities of daily living.

Speech Therapist

Our Speech Therapists will assess your speech problem and will improve your communication ability.

Certified Home Health Aides

Our Certified Home Health Aides will assist you on your activities of daily living such as grooming, bathing, hygiene, eating, and light housekeeping which is supervised as necessary.

Medical Social Worker

Our Medical Social Workers will assess you psychosocial needs and long-term range planning to adjust to the community.

Durable Medical Equipment

We will assess the patient’s need for durable medical equipment such as a cane, walker, wheelchair, and provide you with the highest quality equipment through our DME contractor.
